Do you always know your languages or do you need a moment to switch?

So weird question. I'm very new to the idea of "knowing" another language, and don't currently know one. When you hear something speaking in one of your other languages do you need to take a moment to understand that the person is speaking your other language so you can process it, or do you immediately know what they are saying?
