For language teachers: How do you teach students to talk out loud?

I'm interested in doing some kind of language exchange program. When do you introduce new vocab and phrases vs. sticking to ones they know? How do you give feedback about their mistakes, if at all? How do you assess the abilities of new students and modulate to their level of knowledge? How do you motivate shy students to want to talk and make mistakes? Thanks for helping!
