How do you make GOOD flashcards?

Anyone can make Anki flashcards. But what I want to know is how do you go about making the best possible flashcards? These questions are more to do with the theory and reasoning behind why you do what you do. I'm not asking for a tutorial.

Pictures: How do you use pictures in your cards? Do you only use pictures for physical objects/nouns, or do you try to use pictures in every single note/card possible? Where do you get your pictures from?

Colors: How do you utilise colors in your cards, if at all? Do you color your verbs based on how they conjugate (regular vs irregular), or nouns based on gender (if your L2 has it)?

Phrases: Do you use phrases to remember certain words better, or perhaps to remember how some verbs conjugate? Maybe you use phrases to add context to a word? Or maybe you don't use phrases at all, leaving the refinement of its meaning/usage up to immersion.

Note types: Do you use Anki's predefined note types, or do you make your own custom note types? Do you have different note types depending on the word type (noun, verb, etc.), or do you have one big Swiss army knife note type that you use for everything?

Voice: Do you use audio from movies/tv shows? What about text to speech engines? Or do you do all of your reps in silence?

Card sides: What info do you normally put on each side? What about multi-sided cards? How many sides do your cards have (recognition, production, listening, etc.)?

Your native language: How heavily do you rely on your native language in your cards? Do you try to avoid it wherever you can, or do you use it in every card?

Your target language: How heavily do you rely on your existing knowledge of your target language in your cards? Do your cards build off of each other, or do you make your cards try to exist in a vacuum? How well do you feel you need to know a word before you can use it to help you learn another?

How personal are your cards?: Are your cards filled with in-jokes or personal experiences either alone or with close friends and family? Or do you try to make your cards free from personal context so that in theory anyone can use them?

This post is labeled "Discussion" for a reason. I'm not the only one who can benefit from this information. Maybe someone could reply to you with tips to make your cards better, or maybe the other way around. Because people can have strong opinions on this topic, all I ask is that everyone interacting with this thread be kind and helpful to each other.
